WHO WE SERVE

Carlson Home Hospital School provides temporary instructional services for students living within the LAUSD boundaries in grades TK-12* who are unable to attend their home schools due to medical or psychiatric issues. Face-to-Face teachers provide five hours of instruction each week in two core subject areas, typically English Language Arts and Mathematics. Carlson also has a virtual program, Carlson Home Online Academy (CHOA). CHOA Alternate Home Curriculum offers instruction in a virtual small group setting for up to 2.5 hours per school day. CHOA Secondary offers students in grades 6-12 on the general education curriculum up to 5 hours of instruction per school day

If you feel your child may need Carlson's temporary instructional services, please complete and fax or mail the Carlson Home Medical Referral or Carlson Psychiatric Referral to Carlson Home Hospital School. Our fax number is 818-505-0246. Please attach your child's immunization records to the referral. You can obtain one of these referrals by asking your child's homeschool or by calling the Carlson office. You may also download the referrals from this webpage (please see below).

*Preschool-aged students with disabilities are served by the LAUSD Early Childhood Special Education Program rather than Carlson.  For information regarding LAUSD Home Hospital instruction for preschool-aged students with disabilities, please contact Early Childhood Special Education at 213-241-4713.
